The above listed participants are participating in the Filipino-American Sports Teams of Detroit (FAST-D) sports leagues, its related events and activities, all know as “the Program.” Each individual listed above acknowledges and agrees that:

 

1. I, myself, knowingly and freely assume all risks, both known and unknown, even if arising from the negligence of the Releasees (as described below) or others, and assume full responsibility for my participation and/or my child’s participation; and, 

2. I, myself, and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby indemnify, release, and hold harmless FAST-D, Inc., FAST-D Hoops, Inc., Jack & Patti Salter Community Center, UWM Sports Complex, ReImagine Recreation, its members, directors, officers, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of the premises used to conduct the event (“Releasees”), from any and all liabilities, including, but not limited to, any and all injury, disability, death, or loss or damage to person or property incident to my involvement or participation in the Program, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
